Welcome to Outlive Concierge Medicine (OCM). By accessing or using our concierge medicine services, website, or patient portal, you agree to the following membership terms. Please read them carefully.
1. Medical Services Provided
Outlive Concierge Medicine offers concierge-style medical care, including preventive care, chronic disease management, hormone optimization, and healthspan diagnostics. Our services are provided through telehealth services and in-person visits, depending on your location and clinical need.
OCM operates under a non-insurance model. Services are provided through a membership program. Emergencies and hospital care are not included.
2. Membership and Fees
Membership fees are billed monthly or annually as outlined on our [Pricing Page]. Fees are due in advance and auto-renew unless canceled in writing with appropriate notice. Low-income or barter-based agreements may be considered on a case-by-case basis.
3. Cancellation and Refund Policy
Patients may cancel their membership at any time with 30 days written notice. No prorated refunds will be given for the current billing cycle. Refunds may be considered in cases of billing errors.
4. Telehealth and Communication
By becoming a member, you consent to receive care via secure telehealth services, email, text messaging, and phone calls. While we use encrypted tools, no communication method is 100% secure. You understand and accept these risks.
OCM does not provide emergency care. In the event of a medical emergency, you agree to call 911 or go to the nearest emergency room.
5. SMS Communication Consent
By opting in, you may receive text messages related to:
- Appointment reminders
- Health updates or follow-ups
- Billing notifications
- Important announcements

- SMS Data Protection: Phone numbers obtained for SMS consent will not be shared with third parties for marketing purposes.
- Message Types: Appointment reminders, follow-ups, billing inquiries, or promotions (if applicable).
- Frequency & Fees: Message frequency may vary; standard message and data rates apply.
- Opt-In/Opt-Out: Opt-in by verbal, online, or paper form; opt-out anytime by replying “STOP.”
- Help: Reply “HELP” or contact OCM directly for assistance.

6. Privacy and Security
Your personal and medical information is protected under HIPAA. Patient privacy is a top priority. OCM uses HIPAA-compliant software for all communications and data storage. You agree not to share your login credentials or access your health information on shared or public devices.
7. No Insurance Reimbursement Guarantee
OCM does not bill insurance, Medicare, or Medicaid. Some patients may submit invoices for potential HSA/FSA reimbursement, but OCM does not guarantee eligibility.
8. No Guarantee of Results
While Outlive Concierge Medicine provides comprehensive care using preventive care, chronic disease management, and healthspan diagnostics, results are not guaranteed. All medical treatment involves some degree of risk.
9. Limitation of Liability
To the fullest extent permitted by law, OCM and its providers are not liable for any damages arising from your use of services, except in cases of gross negligence or willful misconduct.
10. Governing Law
These membership terms are governed by the laws of the State of Nevada. Any disputes will be resolved in Clark County, Nevada.
11. Changes to Terms
OCM reserves the right to modify these membership terms at any time. Continued use of our services after changes are posted constitutes acceptance of those changes.
